Assumption of Risk Agreement (please read carefully) The undersigned assumes all risk and responsibility for damage or injury that may occur to the undersigned as a participant in hockey activities or in using the equipment or facilities of the O’Malley Ice Center. Skating Surface: The arena faces a busy schedule and as a result the ice may have standing water, holes, cracks, and rough or soft spots. Our staff will make every effort to provide safe ice. However, players must assume the risk of the foregoing ice conditions. Other Players: The arena assumes no responsibility for the conduct of participants in this hockey program. Players assume the risk of negligent, careless or reckless acts of other players while participating in the league (including before, during and after game times). Referees: All referees for the arena will be certified. Player Equipment: Players are required to wear full protective gear, including certified helmets. See USA Hockey Rule 304 for required equipment. All Players are Required to wear a Face Shield. Duty of Inspection: Players and coaches are responsible to inspect the ice, goals, boards and bench areas (including doors) before each game or other rink activity in order to familiarize themselves with existing conditions. Defects should be reported to appropriate arena personnel. Prohibitions: In addition to the customary rules of ice hockey, participants are prohibited from fighting, roughhousing or other behavior endangering person or property. Participants are prohibited from playing hockey while using drugs or alcohol. The arena is not responsible for lost or stolen articles. O’Malley Ice Center reserves the right to suspend players and/or teams for any amount of time. The arena also reserves the right to move teams to different divisions. Throughout this document ‘arena’ means O’Malley Ice Center and includes agents, officers, and employees of O’Malley Ice Center. I have read, understand and agree to the terms of the foregoing HOCKEY WARNING AND NOTICE and have been given a copy of said HOCKEY WARNING AND NOTICE. I understand that I must have a current Player ID card to participate in league play. Once I have handed my card to the scorekeeper my card will not be returned until the game has ended unless I am leaving early to participate in another game. It is my responsibility to notify the scorekeeper before going onto the ice if I will need to leave early.

Waiver of Liability, Release Assumption of Risk & Indemnity Agreement For and in consideration of participant's registration with O’Malley Ice Center, it affiliates, local association and member teams and being allowed to participate in O’Malley Ice Center events and member team activities, the parent(s) or legal guardian(s) of participant relinquish any and all liability for and cause of action for personal injury, property damage or wrongful death occurring to participant arising out of participation in O’Malley Ice Center events, member team activities, ice skating, the sport of ice hockey, and/or activities incidental thereto, whenever or however they occur and for such period said activities may continue, and by this agreement any such claims, rights, and causes of action that participant may have are hereby relinquished and the participant (or parent(s).guardian(s) does (do) so on behalf of my/our and participant's heirs, executors, administrators and assigns. Participant and/or participant’s parent(s)/guardian(s) acknowledge, understand and freely agree to assume all risks inherent in ice skating, ice hockey and any member team activities, and understanding that said sports and activities involve dangers and hazard risks to participant's person including bodily injury, partial or total disability, paralysis, and death, and damages which may arise there from and that I/we have full knowledge of such risks. These risks and dangers may be caused by the negligence of the participant or the negligence of others, including the "releasees" identified below. It is further acknowledged that there are may be risks and dangers not known to us or are not reasonably foreseeable at this time. Participant and/or participant's parent(s) acknowledge, understand and assume the risks, is any, arising from the conditions and use of ice hockey rinks and related premises and acknowledged and understand that included within the scope of this waiver and release is any cause of action arising from the performance, or failure to perform, maintenance, inspection, supervision or control of said areas and for failure to warn of dangerous conditions existing at said rinks, for negligent selection of certain releasees, or negligent supervision or instruction by releasees. Participant or participant's parent(s)/guardian(s) agree if any claim for participant's personal injury or wrongful death is commenced against releasees, he/she shall defend, indemnify and save harmless releasees from any and all claims or causes of action by whomever or wherever made or presented for participant's personal injuries, property damage or wrongful death. It is the purpose of this agreement to exempt, waive and relieve releasees from liability for personal injury, property damage, and wrongful death caused by negligence, including the negligence, if any, of releasees. 'Releasees' include O’Malley Ice Center its affiliates, its affiliate associations, local associations, member teams, event hosts, other participants, coaches, officials, sponsors, advertisers, owners and operators of the premises used to conduct any event and each team of them, their members, shareholders, representatives, officers, directors, agents and employees. Participant and/or participant's parent(s)/guardian(s) acknowledge that they have been provided and have read the above paragraphs and have not relied upon any representations of releasees, that they are fully advised of the potential dangers of ice skating and ice hockey and any member team activities and understand these waivers and releases are necessary to allow amateur ice skating and ice hockey to exist in present form.
